As a registered NDIS provider under NDIS, we can connect current and potential recipents of NDIS funding with community networks, skill building programs, disability support, and many other culturally appropriate services

Community Nursing Care
Our skilled nurses and support workers aid participants with complex mental health and disability needs. We help transition from hospital to home/residential care under NDIS guidelines. Services include diabetic management, medication oversight, disability care (PEG, Continence, Ventilator, Bowl), pressure/wound care, assessments, and daily in-home health support.
Support Coordination (Level 2 & 3)
Afri-Aus Care’s support coordinators simplify your planning process, offering tailored assistance to match your goals. We connect you with culturally appropriate providers and help you overcome barriers, reducing complexities. Our three areas of support include connections, coordination, and specialist assistance.
Assist Personal Activities
Assisting participant with medication, help with medical appointments,
bathing, dressing and other household tasks.
Community Participation
We support participants to partcipate in the community through various activities. It helps participants solve problems through exchange and sharing of thoughts, a feeling of community belonging and connectedness.
Assist Life Stage Transition
Afri-Aus Care provides short and long term support for life stages and transitions, bolstering participants’ ability to thrive at home and in the community. This includes connections, coordination, accommodation help, life transition planning, mentoring, skill development, decision-making aid, daily planning, and budgeting. Contact us for these services.
Group Centre Activities
We provide support for interactive group activities. We offer recreational and life skill activities tailored for different age groups that are culturally appropriate.
Household Tasks
Our experinced support workers will work with you to achieve your goals in areas of capacity building and skill development. Where you are not reasonably able to complete such activities or tasks our support workers will offer support in those areas.
Development Life Skills
Development life skill support focuses on developing or improving the abilities that can help partcipants live their lives independently and meaningfully.
Assist Travel & Transport
With travel and transport support, individuals are able to travel to places where they can not with other means of transport. The staff and programs at Afri-Aus care are designed to support participants live a meaningful and impactful life.
